Failure modes and effects analysis (FMEA) is a systematic approach of identifying and analyzing the points of failure and determining their overall effects on the system. It is a bottom-up approach that starts working at the lowest level of the system and goes up accordingly. The failure modes and effects analysis shows why and where the failure can occur and how it will affect the system's performance. FMEA can be applied to conception phase, design, process, service and equipment. The steps of FMEA include assigning each component an identifiers, defining functions of the component, identifying failure in each function, describing effect of each failure, estimating probability of occurrence, determining ease of detection, identifying highest risk and mitigating the risk.
