Cause-and-effect diagram is also known as fish bone diagram, Ishikawa diagram, or why-why diagram. It is important to identify and eliminated the causes of the quality problems rather than focusing on the symptoms. A major problem is identified and the causes of that problem are listed so that improvements in the quality can be made. As the name indicates, fish bone diagram looks like the skeleton of a fish. The head of the fish shows the major quality problem, the ribs show the causes of that major problem and the smaller bones refer to the sub-causes. All the causes and sub-causes associated with the major problem are listed until no cause is left, the key causes are identified which must be eliminated, and finally those key causes are addressed.
