A change request is a formal proposal to modify any document, deliverable, or baseline.

Change Request Notes:
During a project, there arise situations when changes in the project are to be made. The changes can be based on different reasons. A customer can initiate changes in the requirements. A contractor can also initiate changes in the project. Also, changes in the cost, schedule or scope can take place. These changes in the project must be formally recorded. A document that requests changes in the project (scope, cost, schedule etc.) is called change request. A change request can be optional or mandatory (according to the contract). Change request can include corrective action (actions that tend to align the performance with the plan if a deflection happens), preventive action (actions that tend to prevent any deflection from the plan), defect repair (actions to convert a nonconforming product into conforming), and updates (changes in the project plan to list the modified ideas).
